Instructions for Collection and Transport

Tubes must be labeled with the following information:

1. Patient’s complete name

2. Clinic Number

3. Location

4. Date of Collection

5. Collector’s I.D.

 

Tubes may be labeled with stamped or computer-generated labels or info may be handwritten. Do not centrifuge. Transport at room temperature. Refrigerate if holding overnight.
