Instructions for Collection and Transport
Specimens will be collected in the ENT Clinic (Dr. Ed McCoul).
Sendout Lab: A physician signature and a patient signature are required on the MicroGen Dx test requisition form, and patient insurance information must be submitted with the sample. Ship at ambient temperature using the MicroGen Dx shipper.
