Instructions for Collection and Transport

Patient should arrive with Myriad sample collection kit.

Collect the tube(s) provided in the kit. Include the Myriad test requisition form if completed by the ordering provider. Make sure the tubes are labeled.

MAKE SURE THAT THE SAMPLE IS KEPT AT ROOM TEMPERATURE.

Place the vial(s) in the enclosed transportation tube with the absorbent strip. Place tube(s) back into test kit box for shipping and transport at room temperature.

If the patient does not provide a Myriad test kit or Myriad requisition form then collect 2 lavender EDTA tubes. Transport whole blood to OC Jeff Hwy CSR at room temperature. CSR Storage: Store at room temperature.
