Instructions for Collection and Transport

Patient should arrive with Myriad sample collection kit.

Collect and label 2 Lavender EDTA whole blood samples. Complete the test request form and barcode label with patient name and or ID and date of birth. Peel label from the request form and place on the vials. MAKE SURE THAT THE SAMPLE IS KEPT AT ROOM TEMPERATURE.

Place the vials in the enclosed transportation tube with the absorbent strip. Place tubes back into test kit box for shipping and follow the steps printed on the box.
