Instructions for Collection and Transport
Tubes must be labeled with the following information:
1.Patient’s complete name
2.Clinic number
3.Location
4.Date of collection
5.Collector’s I.D.
Tubes may be labeled with stamped or computer-generated labels, or info may be handwritten. Do not centrifuge. Transport at room temperature. Refrigerate if holding overnight.
